There is no "insert into images" in your debug output. So it seems that
all your images have found their way into darktable's database already.

One possible cause for your problem is some corruption of darktable's
mipmaps cache. Please delete it and then try again. It can be found in
$HOME/.cache/darktable/mipmaps-*.

Another thing worth trying is go back to commit
9f7ea6b44e0d5cd686b24f685a1c990249ac1c6a. This is the commit just before
I implemented some fixes for bug #9032. Maybe I screwed something
(hopefully not).
