IIRC "history altered" is a crude heuristic that just compares the enabled modules of an image with its default processing. As it was said before, the "darktable|changed" tag is added when you open an image in darkroom (ideally that should only happen when the history changed, it currently isn't added when applying styles, too) and it is removed at export time.
