I'm not sure that this is entirely useful for your purpose, but I'm appending an extract from my book /Suicide and Society in India/ which lists the names and contact details of the suicide prevention organisations which I could locate when the book was published in 2011. Most, though not all--especially in Kerala--are in capital cities.

Crux is that info regarding this should ideally be 'updated' by end users. Which would provide future seekers with up to date info + feedback on reliability. Doesn't need to be rated. Could be form based feedback.

Should be long-lasting, and low maintenance. Responsive. Screen reader accessibility would be a huge plus.

Should be as easy to use as a Crowdmap.

After someone reached out on FB about a suicidal youngster I did a 24 hour dump of some stuff on Google Maps but in the interim have forgotten the codes (24x7x365, emotional vs suicide, defunct etc) and the spreadsheet of information from volunteers wasn't formbased and is therefore chaotic. So don't judge!
