I've parsed the state and districts PDF files to CSVs, of recently released NFHS-5 data (phase-1) here https://github.com/pratapvardhan/NFHS-5
Currently, repo data is available for 311 districts, 21 states. (30 files need parsing or manual entry) Do recheck with source before using this data.
