Start by setting smtprelay.tab with you "trusted" IP set ( or empty it if
you do not have any trusted fixed IP ).
Then use smtp authentication to enable clients relaying.

Very easy. From an untrusted IP and without configuring smtp auth in the
client ( MUA ) try to send a message to an external domain.
