It's not impossible to do it yourself, have a look for the OpenDML 2.0 specification online. MSDN is useful for structure defs and explanations etc. Then you have full control over index structure, insertion of extra data, etc.
However be prepared for plenty of head scratching and gathering bits of different specs together for the audio and MPEG video parts..
