Berend, to reproduce:
- create a database document based on an LDAP address data source
- start the form wizard
- on the first page, select the "First name", "Last name", "Display name",
E-Mail",
"Addresss 1", "City", "ZIP" fields
- Advance to step 5 ("Layout")
- chose the "In blocks - Label above" layout
=> some fields are positioned above others
