Here are the steps to reproduce. I've added my own translations from Dutch to English - these might not be the official translation. Preparations: - I've created a new OpenOffice Base file. - After that I've added a new table based on the template Adressen (addresses in English). (I'll add the file to the bug report) - I try to create a new form using the wizard at the tab Formulieren (Forms): -'Formulier maken met assist' (create from using wizard) -Choose any fields (I tried a few combinations) -Choose 'Voltooien' (Complete) -Now OpenOffice crashes immediately. I tried the above steps many times using different combinations of fields, all in the same result. Also not hitting 'Voltooien' immediately but going though extra steps doesn't help either.
