I had been waiting to see if the problem would be cleared up in version 3.1, but having just installed Novell's Go-OO version of OpenOffice 3.1, I see that the issue persists. My forms are now coming up OK, but, if memory serves, I ended up redesigning all of them after I got the zoom-slider situation sorted out. I used the design view rather than the wizard.
