and likewise for

        vfprintf (DBILOGFP, ...)        => PerlIO_vfprintf (DBILOGFP, ...)
        fflush   (DBILOGFP)             => PerlIO_flush    (DBILOGFP)
           note here that it is _flush and not _fflush
