feature: Add option to disallow UPDATE to overwrite an existing primary-key
row
This adds a dbm_updatable_key attribute, with the following values:
- 0 = no change, default behavior as is
- 1 = warn on duplicate key but continue as usual
- 2 = croak on duplicate key

fix: add missing import of refaddr to DBI::DBD::SqlEngine
Debugging DBD::DBM for the dbm_updatable_key tests triggered an error
about a missing refaddr method.
