Paul, There is a small typo in the CREATE INDEX added here.
As of 2751 of the 2_2_branch there is an extra Right Parenthesis in
create_tables.pgsql:
CREATE INDEX dbmail_headervalue_3 ON
dbmail_headervalue(substring(headervalue,0,255)));
