I've just about finished this code for now.

What I've done so far:

I've cleaned up the migration script a little, and added cleanout
statements for all cache tables;

I've also added a hash field to dbmail_headervalue just like mimepart
has. The idea is that this will allow fast lookups on headervalues.

After using the migration, all that is needed is dbmail-util -by to
reload the cache tables.

I've called this topic branch 'header', and it's on dbmail.eu

Todo: migrations for mysql and sqlite, plus testing, testing....
