I currently use Postfix + Amavisd-new(w/Mail::Spamassassin) + DBMail and
this works great.  the Mail::SpamAssassin module is controlled within
amavisd-new and I'm using antivir which is controlled by amavisd-new as
well.  A very mangeable solution.
