The answer is: it depends on how you set it up. Look under /etc/dbmail.conf and read the comments next to sqlport= and sqlsocket= it should explain everything.
Alternatively, you can do a netstat -a to see if you have localhost connections to the mysql server. Just stop and start dbmail.
