Perhaps try pam_mysql? And make qmail use pam for smtp-auth (Is that possible, 
I'm a sendmail person myself).
I think it has it's own table schems, but I'm sure that with a little hacking, 
you could make dbmail add users to it's tables too, or make pam_mysql use 
dbmail's tables.

Also, dbmail supports pop3/imap-before-smtp, you should look into that too.
