Install dbmail from testing. Or better yet, if you want to apply John's patch you'll have to use the unstable packages available at:

deb http://debian.nfgd.net/debian unstable main



apt-get source dbmail-mysql
For this add also:

deb-src http://debian.nfgd.net/debian unstable main

and do

apt-get source dbmail
apt-get build-dep dbmail


save patchfile in debian/

Well actually in debian/patches/

You will also have to edit the file debian/patches/00list and enter the name of John's patch. There's a small chance this patch will break on the preforking patch.

dpkg-buildpackage

and keep your fingers crossed.

Since you're running woody I'm pretty sure this will not work however. The debconf stuff *will* break, and some build-dependencies may also be unavailable. Consider upgrading to testing.
