You're talking about IDLE capability, also described as non-sollicited
server response. Dbmail doesn't support that, and won't in 2.2. It's a
planned development target during 2.3.x because doing it right requires
a complete redesign of the server code handling concurrent client
connections.

Of course, email does arrive automatically, but the status change in the
mailbox is not updated in the client until the clients requests a status
update for the mailbox. With IDLE support, the server can send
incidental updates for mailboxes to connected clients, and the client
will handle those updates as expected. All serious imap servers should
support IDLE. It's definitely a cool feature and I intent to see it
implemented in dbmail.
