Have you run dbmail-util -by? You'll need to do that to get the
headercaches updated and other internal flags correct.

Also, are you using Martin Hierling's ebuild in the Gentoo Sunrise
overlay?

I have run dbmail-util -by - the e-mails that were received before the
upgrade were fine it was only the e-mails that were received after the
upgrade that have a problem so I suspect it is with dbmail-lmtpd and
dbmail-smtp.

I was using the ebuild in the normal Gentoo portage repository for 2.1.0 -
for the new versions I was compiling from source.
