Have we seen any dbmail logs at trace level 5 yet? A key question is what your database is doing. If there's some point where the database reports back to DBMail that a query wasn't executed, but DBMail goes ahead and reports that the message is received, then we have a major problem.
Let's also see what your MTA is doing (was it Postfix?). Hopefully something sane in this situation, too.
