I see these myself only with Outlook, where outlook issues IDLE without first
selecting a mailbox. That is a violation of the IMAP RFC which flows from
Outlook's multi-threaded nature:
- thread 1 connects, logs in and selects a mailbox
- thread 2 connects, logs in and issues a command against the mailbox selected
by thread 1.
A level=4 should be sufficient to confirm this.
