I believe the files are MD5 hashed or similar, coupled with file length
rather than name.

If you send two identical emails (the same messege-id) to the same
account, then only the first one will show up in your inbox. The second
one will be silently dropped. Saving you the hassle of looking at the
same email twice.
The new storage model in 2.3 helps, if two accounts receive the same
.zip file as attachments to some mails, then the .zip attachment would
be stored in the db only once. Saving disk space on the server.
Totally different features.
