You could do this with a script or small LMTP server that sits between the MTA and DBMail, and removes certain MIME attachments and replaces them with e.g. a url to a web server where the attachment is now stores. I know of at least one commercial product that does this, and there might be some open source code to do the same. DBMail does not itself natively support doing such a thing.
