Yes. VBox is accessible via the CLI (VBoxManage), but also the GUI if you 
install qt-at-spi. The dialogs are accessible, except the main window as the 
machine names use not Qt conventional objects.

The VM will not be more or less accessible. As for a physical machine, their 
accessibility will depend on the OS and wether the AT is installed or not. 
However you have an access to the VM features accessible via a binding, e.g. 
right ctrl+f (follscreen), right ctrl+D (shared folder), etc). finally, if you 
do alt-f4 inside a VM but the focus is not captured, you will get the window 
with capabilities when VM is paused.
