Comments/Problems:

This happens on 2.4 or 2.6 Install kernels with acpi on or off, using either
installer RC1 or RC2

Using Guided Partitioning failed every time, when formatting Root partition
it hung at 33%. When setting up partitions manually it failed formatting at
33% with "Failed to create a filesystem"
To bypass this problem, I booted machine from Powerquest Partition
Magic 8 Boot floppies and set partitions up (Created & formatted). Then
started install again and installed onto the pre-existing partitions keeping
data and file system intact.
After this it failed at 35% Installing the base system.
