To replicate: start irssi-gnome. An entry appears for it in the
panel. Immediately quit irssi-gnome. A dialog box appears saying:
irssi applet appears to have died unexpectedly.
Reload this applet?
Reloading appears to have no useful effect (and isn't what you want
anyway).
