There are currently at least two reports floating around that seem to have
the same root cause: the way devmapper devices are created for lvm
partitions. The relevant reports are:

These recent issues seem to have been triggered by a recent change in udev
on request of the Gnome team:

Although this is a lot older, it seems related:

Similar issues are also seen in other distributions:

IMO the issues listed above together are serious enough that they should
be fixed, or at least addressed, before the release of Etch.
I'm filing the BR against lvm2 as that is the package most responsible for
the lvm setup in Debian, though fixing this probably involves udev as
well.
IMO 401393 and 402511 are serious enough that reverting the change from
392623 should be considered as one option.

The main intention of this BR was to get some clarity in the regressions
seen because of the change in udev.
Further analysis in http://bugs.debian.org/406697 has now shown that the
reason the /dev/dm-* devices are not removed, is the fact that udevd has
already been killed by the time the crypto and lvm scripts are run on
shutdown and thus the events created by the kernel are no longer
processed as they would be if the system was running normally.
This makes the issue a lot less structural than initially feared (at least
by me) and thus this "umbrella" bug report can be safely closed.
