I always encounter this error when I:
RT -> RTFM -> Configuration -> Classes -> New -> Submit
I have no defined classes, and it was a pretty straight forward install.
RT is working very well, but RTFM on the other hand is not.
After hitting submit, I see this error:

Internal Error: Couldn't execute the query 'INSERT INTO FM_Classes
(LastUpdatedBy, SortOrder, Created, Name, Description, Creator,
LastUpdated) VALUES (?, ?, ?, ?, ?, ?, ?)'ERROR: relation "fm_classes"

Hi,
the newest rtfm package (2.2.1-1) tries to automatically initialize
the database if possible, and its debconf configuration dialogs point
to README.Debian for more information. Earlier versions indeed needed
manual rt-setup-database invocations.
I'm closing this bug. Please let me know if this isn't satisfactory.
