  I've been trying to use html2text to convert a DocBook-generated page
to a text file.  It almost works, except for a few non-ASCII entities
that html2text wants to leave in the result: &#8220;, &#8221;, and
&#8212.  &#8220 is an "opening quote", and `` is a sensible replacement
for it; &#8221 is the paired "closing quote", and '' is a sensible
replacement for it.  &#8212 is a "long dash", and -- works well.

  It would be nice if these could be added to ascii.substitutes; I tried
sed'ing them out, but that messes up tables that contain them.  As a
longer-term request, being able to override ascii.substitutes at runtime
would be ideal.
