Section 6.5 of the Administrator's Guide refers to PAM_ITEMs and lists
PAM_RHOST as an example, but the Administrator's Guide never says what 
a PAM_ITEM is or lists possible items.

The PAM documentation has been substantially rewritten since this bug was
filed, and I no longer see any references to PAM_ITEM at all in the current
version of the system admin guide.  Closing this report.
