account is disabled, you may get the wrong error code from
pam_acct_mgmt.

I believe this bug is fixed in the current version of pam; at least, I can't
see any path by which an expired account would return PAM_NEWAUTHTOK_REQD
instead of PAM_ACCT_EXPIRED when the expiration date is past.
