The main issue is because ZBX_SERVER and ZBX_SERVER_PORT is not defined
when script.inc.php is executed.
This variables should be defined in conf/zabbix.conf.php.
Checking source code and zabbix forums (see below), you will notice that
these vars are not defined/created when zabbix Debian package is installed.

A good approach would be to ask in the debconf these 2 vars and include
them in the creation of the configuration file.
