Ok, this is an old known annoying bug with either isapnptools
or the PnP hardware of Creative Soundblaster AWE64 (Gold) cards...
It is documented in the Soundblaster-AWE mini-FAQ. Here it goes:

pnpdump provides this dump for the MIDI-support of AWE64:

What it misses are two additional lines for each configuration
alternative. Without them, there MIDI-support won't work. Here's

I suggest
  1) If there's a bug in isapnptools causing this
     (there might be since it is detected smootly in Win95/98/..),
     fix it.
  2) If not, add an "exception" database where  details about 
     "broken" PnP cards are stored. This database should be used by
     pnpdump in realtime.
