To reproduce:
1) Connect one monitor (for me, a 24" Dell) to the external DVI port
2) Run nvidia-settings, select the "X Server Display Configuration"
entry, and click on "Detect Displays"
3) Configure the 2nd display for TwinView, Apply the changes, and exit
nvidia-settings
4) Run nvidia-settings, select the "X Server Display Configuration"
entry, and disable the 2nd monitor.  Apply the changes and exit.
5) Disconnect the monitor
6) Connect a different monitor to the external DVI port (a 19" LG in my
case)
7) Run nvidia-settings, select the "X Server Display Configuration"
entry, and click on "Detect Displays".  The old Dell monitor is still
displayed in the Layout pane.  I have to quit nvidia-settings and
restart it in order to configure the new display.
