Hi,
during a test with piuparts I noticed your package modifies shipped
files. As /usr/lib/swi-prolog/library/INDEX.pl is regenerated during
postinst, there is no need to ship this file.
Instead it needs to be removed in the postrm script (during remove).

from the attached log (scroll to the bottom...):
