There is not much information I can give about this error (exactly what 
conditions will trigger it, etc.)
However, this has occurred before and I had tried copying the actual data files 
onto an ext2 partition to no avail,
so this does not seem to be related to trying to make an isofs from an smbfs..

My latest mkisofs run produces

   Using ARTWO003.HTM;1 for  ./To Allison 2/To Patreat/PhaseIIfunding/ARTWORD'S 
MISSION.htm (Artword Theatre Specifications.htm)
   mkisofs: Error: './To Allison 2/To Patreat/PhaseIIfunding/ARTWORD'S 
STRATEGY.htm' and './To Allison 2/To Patreat/PhaseIIfunding/ARTWORD'S 
STRATEGY.htm' have the same Rock Ridge name 'ARTWORD'S STRATEGY.htm'.
   mkisofs: Unable to sort directory ./To Allison 2/To Patreat/PhaseIIfunding

and no ISO image is produced.

The strange thing is that the fatal error is in the form

   'bar/foo' and 'bar/foo' have the same Rock Ridge name 'foo'

which does not make any sense at all.

Some (long) time ago you filed this bug against the cdrecord/mkisofs/cdda2wav
package. In the meantime the package has been replaced by
wodim/genisofs/icedax.

If you can reproduce the same issue with one of the replacing packages (as
released with Debian 7.0 wheezy or later) please reopen and reassign the bug to
the new package and use

    reportbug <new-package>

to send a follow-up containing updated system information.


Please note that unless it is reassigned to an existing package further
information sent to this bug is likely to remain unread, because the bug is
currently not assigned to any existing package.
