It tries the default, waits the timeout, and then loops around to try 
the default again. As a result, it's impossible to load any image except 
the default if the default image loads too quickly to abort. (For 
example, if 'disk' is the default, you hardly get any chance at all to 
abort).

The fix should be that pxelinux should wait the timeout, _then_ try the 
default choice.

I reported this bug in error. One must specify "PROMPT 1" in the config
file to get a prompt and timeout delay before trying the default. It's
in the pxelinux documentation.
