Hi,
since the recent X.org transition (I suppose), the link
/usr/include/X11 -> /usr/X11R6/include/X11 no longer exists.
Hence, with the default ImagePath in fvwm being
"/usr/include/X11/bitmaps:/usr/include/X11/pixmaps", the icons from the
fvwm-icon package are no longer accessible in fvwm without explicit
manipulation of ImagePath (or explicit specification of the path).
Would you please consider moving the pixmaps from /usr/X11R6/...
into /usr/include/X11/pixmaps/.
