ifrename causes a kernel panic during boot if the following conditions
are verified concurrently:
1) a PCMCIA wireless card is inserted during boot;
2) ndiswrapper is configured for the card and the ndiswrapper ;
3) the interface's configuration is missing in /etc/network/interfaces
The crash is averted if the card is inserted into the slot after the
boot process (specifically, after ifrename was executed).

Kernel panics are always bugs in the kernel, not in userspace utilities.
Either the ndiswrapper is buggy, the Windows driver it loads is, or:

This "kanotix" kernel is patched in a way that breaks it.
If you can reproduce this bug with an official, unpatched kernel, or
with one of the kernel packages from Debian main, you should reopen this
bug and reassign it to a kernel package, but in any case this is not a
bug in ifrename.
