  debian/copyright doesn't contain Upstream URL where you
obtained the source and upsteam author's name.  This is the
policy violation at Debian Policy Section 12.5.

     In addition, the copyright file must say where the upstream
   sources (if any) were obtained. It should name the original
   authors of the package and the Debian maintainer(s) who were
   involved with its creation.

This bug was fixed in an NMU, but never closed. I'm closing it with the
correct version so that its status can be properly version-tracked.

For reference:

j-cvsbook (1.12-3.1) unstable; urgency=low
 .
   * NMU.
   * Drop pdf support, for I don't know the way to make Japanese PDF from
     texinfo with only DFSG-free package.  So, I install Postscript format
     instead. (Closes: #189912).
   * Add upstream URL and upstream author's name to debian/copyright.
     (Closes: #214206).
   * Update Standards-Version.
