Somehow my system got installed with update-inetd but not inetutils-inetd. I wanted to turn on lpd support so installed cups-bsd. update-inetd ran without apparrent errors when I did # dpkg-reconfigure cups-bsd to enable the lpd server, but there was no inetd.conf file to append to and none was created. No error was indicated. Once I installed inetutils-inetd and reconfigured cups-bsd again, things worked as expected. Possibly this is actually a bug in update-inetd but I'll leave that to your discretion. I'm filing this against cups since it provides the daemon binary program, /usr/lib/cups/daemon/cups-lpd.
