I've had this problem too. It is not limited to Firefox. You can use alt+tab to get back to your window. I remember reading a massive kde bug report about it early last year. Something to do with apps unexpectedly changing something about their own window state while in the background which causes the panel to lose track of it.. If you can come up with steps to reliably reproduce the problem, it would be greatly appreciated by upstream as they have tried to fix this numerous times in the past.
I've had this happen to me, maybe a dozen times in the past month, but it isn't reproducible even a single second later. I'll keep digging for that bug report.
