Recently my orinoco-based card stoped being brought up. When looking for
the problem I discovered the following in my /etc/default/pcmcia:

# If REFRAIN_FROM_IFUP is set to yes, cardmgr will not bring up
# network interfaces. They should be brought up by hotplug instead.
# Since this is an upgrade from an old version of pcmcia-cs, it
# retains the old behaviour. Please read
# /usr/share/doc/pcmcia-cs/README.Debian and then uncomment the
# following line.
#REFRAIN_FROM_IFUP=yes

However, that does not work (the interface is not brought up). When
reading the README suggested, the solution there does not work either:

  mapping hotplug
        script grep
                map eth0

A hint as where to find solution is in /usr/share/doc/hotplug/README.Debian
 
 * To make it enable for hotplug to control network interfaces,
   you list such interfaces on allow- stanzas in /etc/network/interfaces.
   see interfaces(5), ifup(8) and Bug#303383

Finally the manpage interfaces(5) recomends:

allow-hotplug eth1

This finally works.
