Hi Vladimir, your description suggests that the problem lies in cups-filters. Evince converts documents to PS before sending them to cups-filters, which avoids some of the bugs related to forms. I could open a wishlist item for an optional PS-conversion as Evince does, but I think this would be way too much work for such a small project to shoulder in the nearer future.
