some additional questions, after reading your logs more carefully.

Your setup is apparently able to send chipcard commands to the card, so
I would assume that the package libchipcard-libgwenhywfar60-plugins is
installed.
The log mentions a file /usr/share/chipcard/drivers/cyberjack_pcsc.xml
which might indicate that you have a Cyberjack chipcard reader. Is that
true?
If yes: Do you know what Cyberjack driver version is installed on the
reader? There are some known issues with recent Cyberjack driver
versions, as has been reported also in some online banking forums:
http://onlinebanking-forum.de/forum/topic.php?t=17481 (sorry, in German
language only). Apparently the driver version "SP03" is known to work
well, so in case you have a newer driver version, you might consider to
downgrade it...
