Can you attach examples of html, html5 and xml files that will produce the error when run with emacs -q? When you say "insert a tag", do you mean with C-c C-i or from some menu item? While in the buffer with the file that generates this error, can you choose the menu item SGML/General DTD info and add the output to this bug report?
