I have a debmirror running here which is mirroring Ubuntu and Debian. Both distributions recently added dep-11 metadata to their main archive mirrors (dep11/Components-* and dep11/icons-*), which clients download by using apt's new Acquire::IndexTargets feature. This relies on the files being present in the archive's Release file. debmirror doesn't mirror these files, but it does mirror the Release file verbatim - this means that apt thinks that the archive serves the file, but gets a 404 when it tries to download it.
