there are packages (dbus, I'm talking about you!) that suggest a reboot after a package update because they don't feel like being able to be updated without a reboot. needrestart should detect that state and report it, otherwise the suggestion to reboot is gonna be missed by many local admins. Dbus touches /var/run/reboot-required in that case, and the comments in dbus' postinst suggest that there are already packages like unattended-upgrades that use that file to suggest a reboot. Implementing this in needrestart should be easy enough, I hope. Thanks for considering this.
