The only statement (other than the general statement of intent) that
applicants have to sign is related to the SC/DFSG/DMUP:

I agree to uphold the Social Contract and the Debian Free Software
Guidelines in my Debian work.
I have read the Debian Machine Usage Policy and I accept them.

I can't seen any sign of a link to the CoC or Diversity statements as
part of the nm.debian.org site.
