As a first step, the TOC output from a
readcd -clone
run (appears on stderr) from an original CD (not a copy) would help.
Note that you should read the man page first and use supported dev= arguments
only.
Shut down any unfriendly (and thus broken) programs before inserting the
CD (e.g. hald) and do not run any other program that accesses the CD or the
CD TOC before running readcd.

This is definitely not true.
Since 2003, cdda2wav is the most advanced and most reliable DAE known tool on
UNIX. And note: cdparanoia is Linux only.
